Safety Observation and Near Miss Report Form
A safety observation can be any observation involving safety – catching a co-worker doing something right and/or any identified area for improvement. A near-miss is a potential hazard or incident that has not resulted in any personal injury or property damage. Some examples of near-misses are unsafe conditions, improper use of equipment, use of faulty equipment, not following proper procedure, etc. It is everyone’s responsibility to report and correct any of these potential hazards immediately. Please use this form to report safety observations and near-misses and assist us in preventing future incidents and making Loveland Ski Area a safer workplace. This form can also be used to document positive aspects of our safety program and recognize employees for safe acts.
